Report Hurricane Lowell Damage to PHCA

If you have not already, first report your property damage to the Kaua'i Emergency Management Agency.

Once you've reported to KEMA, please use this form to submit a brief description of the type and severity of damage to your property to PHCA. Your report may include damage to your house or other structures on the property (walls, fences, accessory buildings, etc.). You may also inform us of large downed trees.
